Payment Policy: For Pleasant Stick Mobile Lab LLC
1. Fee-for-Service Platform:
Pleasant Stick Mobile Lab LLC is an independent "fee-for-service" platform. We do not bill your insurance for our services.
2. Payment Method:
For each ordered visit, you will be charged a one-time "fee-for-service" directly. We currently accept Credit Card and Debit Card payments.
3. Insurance Information:
Your insurance information must be included on your physician requisition for your specified laboratory to process your specimens.
4. Completing Requisition:
Before uploading your documents, please ensure that your requisition is properly completed with the necessary insurance information. Failure to provide accurate and complete information may result in delays or difficulties in processing your specimens.
5. Payment Due Date:
Payment for our services is due immediately upon receipt of the invoice or booking. It is important to settle your payment promptly to avoid any disruptions or delays in the processing of your samples.
6. Payment Confirmation:
Once your payment is received and processed, you will receive a confirmation email or receipt. Please retain this for your records.
7. Refunds and Cancellations:This is a Premiere Concierge service
If scheduled service is canceled at least 24 hours prior to the appointment , NO Charge.
If scheduled service is canceled after 24 hours of the appointment the customer/patient will be charged 50% of the full cost of the services.
If the customer/patient is not at the scheduled location , or the appointment is canceled within 2 hours or less of the appointment time, the customer/patient will be charged full cost of the service.
8. Disputes:
In the event of any disputes regarding payments or invoices, please contact our customer support team immediately. We will work with you to resolve any issues satisfactorily.
9. Confidentiality:
All payment information provided to us will be treated with the utmost confidentiality and used solely for the purpose of processing your payments. We adhere to strict privacy and security practices to ensure the protection of your personal and payment information.
10. Changes to the Payment Policy:
Pleasant Stick Mobile Lab LLC reserves the right to make changes or updates to this payment policy at any time. Any modifications will be communicated to you through our website or via email.
